We want everyone to have the best possible quality of later life and we know that supporting the vulnerable people you are responsible for can take considerable time and expertise. That’s why we provide an end to end care and wellbeing service to support your vulnerable clients.

We provide highly personalised care and wellbeing plans tailored to your clients, and as much support as is needed. We can manage all aspects of care at home as we are CQC registered and have our own paid carers. We also work with older people living in care homes.

We are a multi-disciplinary team, and we offer other services including mental capacity assessments , COP3 assessments for Deputyship and certificate provision for Lasting Power of Attorney.

Our support is underpinned by a cash flow model to ensure plans are sustainable, which can be created by a separately appointed firm of independent financial planners. Together we can facilitate the legal, financial and care and wellbeing services clients need, ensuring that later life is as safe, secure and meaningful as possible.

We operate in the South East of England (Kent, Sussex, Greater London and parts of Surrey).

Navigating social services

We offer support to families to negotiate the maze of social services. We are a multi-disciplinary team and we can:

 

  • Help complete attendance allowance forms. 

  • Scrutinise reports by social workers and offer feedback.

  • Offer advice about how to deal with disputes with social services. 

  • Offer support and advice about eligibility and referrals.

  • Be a point of liaison with social service or care homes.

  • Write reports or assessments in support of eligibility for services. 

  • Make referrals on your behalf.

  • Help look for care homes or care agencies.

Plan with Care can provide fixed cost mental capacity assessments in Kent.

Ensure, enhance, exceed

Our work is based around three Es – ensure, enhance, exceed. The services we provide are designed to complement and enhance those that solicitors normally provide to their clients. We use a 0–10 scoring system for each person we work with to enable you to monitor clients’ health and wellbeing without needing to get involved in the details.

Our Five-Step Review Process

Our ongoing review process provides you with an audit trail of how your clients' health and wellbeing is managed and supported with their decision making. This protects your clients, as you are quickly made aware of any issues. It also protects you, as it enables you to confidently answer court of protection enquiries, provide independent justification and historical explanations for decisions in the client’s best interest, as well as defend arguments for expenditure.

 

We meet regularly with your clients to assess their wellbeing, checking their care and wellbeing plans are up to date and identifying any opportunities to improve quality of life. We carry out a formal assessment with a written report at least every six months, and an informal assessment every time we visit. 

 

We keep in touch with you throughout, informing you of any changes, and immediately alerting you to any critical issues. Additionally, our solicitors’ app gives you access to all records whenever you need them
