
Wellbeing Support Subscription
Overview
The Wellbeing Support Subscription from Plan with Care has been designed to provide professional attorneys and deputies with support to assure and enhance P's care and wellbeing. Our service ensures that any concerns are identified early, and actions are taken to enhance quality of life. The plan is offered in two tiers—Basic and Standard—to suit differing needs, both providing regular contact and reporting, the only difference being the number of check-in calls (4 or 12 pa).​
Core Features:
Annual Checkup Visit:
• A comprehensive annual visit, approximately one hour in duration, includes an in-depth assessment of physical, emotional, and environmental wellbeing. Conducted in our usual relationship-focused manner, underpinned by wellbeing practice.
• A detailed two-page Wellbeing Assurance Report will be provided, including:

Summary of current situation.

Highlights of issues observed or reported by the individual.

Communicating specific needs or requests from the individual.

Suggestions to improve overall wellbeing.

Reports are shared via email and the Flourish App for ease of access.
Regular Check-In Calls:
• Scheduled half-hour check-in calls provide ongoing monitoring and support, either
with the individual or their care provider.
These calls help:

Monitor the individual’s wellbeing.

Identify and address emerging concerns.

Provide oversight for individuals in care homes by ensuring care quality is

maintained.

Subscription Details:
Basic Level Subscription:
- Features:
- Four half-hour check-in calls per year (one per quarter).
- Comprehensive annual checkup visit with accompanying Wellbeing Assurance Report.
- Reports accessible via email and the Flourish App.
- Cost: £30 per month (inclusive of VAT and travel).
Standard Level Subscription:
- Features:
- Twelve half-hour check-in calls per year (one per month).
- Comprehensive annual checkup visit with accompanying Wellbeing Assurance Report.
- Reports accessible via email and the Flourish App.
- Cost: £50 per month (inclusive of VAT and travel).

We also offer Enhanced subscriptions with additional support from your Health and Welfare consultant being provided on a time basis of £90, and any admin support for £30 ph.
