We’re looking for a Care and Wellbeing Consultant to join our growing team in the South East of England. The role involves working with older people, their families and representatives to enable them to not just survive, but thrive.

The role will involve:

  • Carrying out in-depth written wellbeing assessments for our new clients, including evaluating their environmental, emotional and physical wellbeing.

  • Devising care and wellbeing plans that set out a range of interventions, from fundamental safety measures to highly creative and specialist services that will maximise wellbeing.

  • Providing continuous assessments of client wellbeing, and taking immediate action to improve quality of life.

  • Reacting promptly when a family contacts us for support in a crisis situation.

  • Providing ongoing support for families and representatives when needed.

We want to work with people who are:

  • Passionate about empowering people and making a positive difference.

  • Creative and open to new ways of doing things.

  • Willing to go above and beyond when it comes to ensuring older adults can live the best version of their life possible, no matter their background or resources.

We’re looking for someone with:

  • Excellent interpersonal skills.

  • Extensive experience in client and case management.

  • A background in working with older adults and those living with dementia.

  • A strong understanding of how to liaise with a diverse range of external professionals to ensure our clients receive the best possible care.

  • A desire to be part of a dynamic and ambitious team that prioritises our own wellbeing, as well as the wellbeing of the people we support.

Pay: £26,000–£36,000 pro rata (depending on experience) plus pension.

Hours are negotiable, either in a permanent role or on a freelance basis. We are supportive of flexible working arrangements for this role, as it is more important to us that we find someone who is interested in really pushing boundaries to make a difference than being governed by hours.
